    ARTHFREE ............. Product of EVA - Pharma



    Composition:

    Arthfree 100mg

    Each film coated tablet contains: Leflunomide 100 mg

    Arthfree 20mg

    Each film coated tablet contains: Leflunomide 20 mg


    Properties:

    Arthfree (leflunomide) is an isoxazole immuno-modulatory agent, which inhibits dihydroorotate dehydrogenase (an enzyme involved in de novo pyrimidine synthesis of the cells involved in the pathogenesis of rheumatoid arthritis) and has antiproliferative and anti-inflammatory effects. Through that Arthfree is suitable for patients with rheumatoid arthritis where it:

    -Reduces signs and symptoms of rheumatoid arthritis in the form of reducing the pain, tenderness, inflammation, edema and swelling of the joints, as well as, decreasing morning stiffness

    -Inhibits structural damages of the cartilage and joints through decreasing erosions of joints and joint space narrowing

    -Improves the physical function of the patients manifested by improvement in the patient ability to perform daily activity such as dressing, arising, eating, walking, hygiene, reach and grip

    Arthfree is ****bolized, following oral administration, to an active ****bolite, which is responsible for essentially all of its activity. The peak level of the active ****bolite of Arthfree, occurs between 6 - 12 hours after oral dosing and it is extensively bound (>99.3%) to albumin. This active ****bolite is eliminated by further ****bolism and subsequent renal excretion as well as by direct biliary excretion

    Arthfree (leflunomide) effects are usually evident by 1 month, stabilized by 3 - 6 months, and continued throughout the course of treatment

    Indications:

    For the treatment of rheumatoid arthritis

    Dosage:

    • It is recommended to initiate therapy with a loading dose of one Arthfree 100 mg tablets once per day for 3 days followed by a maintenance daily dose of one tablet of Arthfree 20 mg tablets once daily

    • If dosing at 20 mg/day is not well tolerated clinically, the dose may be decreased to 10 mg daily

    Precautions:

    • Therapy with leflunomide might be associated in some cases with mild elevation in liver enzymes (twofold) and this elevation returns to normal with continuation of therapy. However if the elevation is too high or if levels of liver enzymes are not normalized along therapy, especially in case of concomitant administration of other drugs that affect the liver, dosage adjustment or discontinuation of therapy is advisable. In most of the cases, it is preferable to have a baseline measure for the liver enzymes levels

    • Caution should be taken when leflunomide is administered to patients with renal insufficiency. The use of leflunomide in patients with hepatic insufficiency is not recommended

    Side Effects:

    Arthfree therapy is tolerable in most of the cases although some adverse reactions in the form of diarrhea, alopecia, and skin rash have been associated with leflunomide therapy but they were not frequent & discontinuation of therapy was not considered mandatory in most cases

    Contra-indications:

    • Hypersensitivity to leflunomide

    • Pregnancy and lactation

    • Not recommended for patients less than 18 years

    Drug Interaction:

    • Co-administration of Arthfree with rifampicin increases peak level of the active ****bolite of leflunomide

    • Co-administration of Arthfree with hepatotoxic drugs (methotrexate) increases the risk of hepatotoxicity

    • Co-administration of cholestyramine & charcoal with leflunomide result in a rapid and significant decrease in plasma M1 (the active ****bolite)

    • In vitro studies, M1 was shown to cause increases ranging from 13% to 50% in the free fraction of diclofenac, ibuprofen & tolbutamide in the clinical range

    Package:

    Arthfree 100 mg tablets: Box of 1 strip of 3 tablets

    Arthfree 20 mg tablets: Box of 1 to 3 strips, each of 10 tablets

    Properties and Mode of Action

    ALZENTAL, a benzimidazole carbamate derivative, is an anthelmintic with activity against most nematode and some other worms. Activity against some larval stages and ova has also been demonstrated

    ALZENTAL’s anthelmintic activity is considered to be dependent on the inhibition or destruction of cytoplasmic microtubules in the worm’s intestinal or absorptive cells. Inhibition of glucose uptake and depletion of glycogen stores follow as do other inhibitory effects leading to death of the worm

    Indications

    ALZENTAL is a broad spectrum anthelmintic used in the treatmemt of single and mixed infections as

    Enterobiasis

    Ascariasis

    Hookworms

    Trichuriasis

    Strongyloidiasis

    Taeniasis Capillariasis

    Cysticercosis

    Toxocariasis

    Trichostrongyliasis

    Trichinosis

    Echinococcosis (Hydatid disease)

    Dosage and Administration

    Adults and Children Above 2 years of age

    Enterobiasis, Ascariasis, Hookworm, Trichuriasis

    400 mg as a single dose
    Treatment may be repeated in 3 weeks

    Cysticercosis

    15 mg/kg body weight/day for 30 days

    Strongyloidiasis and Taeniasis

    400 mg is given daily for 3 consecutive days
    This may be repeated after 3 weeks if necessary

    Capillariasis

    200 mg twice daily for 10 days

    Echinococcosis (Hydatid disease)

    (Over 60 kg body weight): 800 mg/day in divided doses for 28 days
    This is followed by 2 weeks drug free interval, to be repeated for 2-3 cycles
    For inoperable cases up to 5 cycles may be given
    (Under 60 kg body weight): 10-15 mg/kg body weight daily for 28 days

    Children Up to 2 years of age

    Enterobiasis, Ascariasis, Hookworm, Trichuriasis

    200 mg as a single dose
    Treatment may be repeated in 3 weeks

    Precautions

    ALZENTAL may cause elevation in hepatic enzymes

    Patients receiving high doses of ALZENTAL, such as those with Hydatid disease, should be supervised closely with blood counts and liver function being monitored

    Contra_indications

    Pregnancy

    Hypersensitivity to Albendazole

    Side effects

    Incidence is rare and reversible: e.g.nausea, vomiting, diarrhoea, leucopenia, elevated transaminases

    ALZENTAL Tablets : Box of 1 blister of 2 tablets
